Union Hospital Recombinant Zoster Vaccine (Shringix) - 2 Dose

This vaccination plan requires a physician's assessment of suitability. If the physician determines that the vaccine is not suitable, the client will be required to pay a consultation fee of HKD370, and the remaining balance will be refunded.
The recommended interval between the two doses is 2 to 6 months. The second dose must be completed within 6 months of the first dose; failure to do so will result in the plan being forfeited.
You must receive and complete the first dose within 6 months of the confirmed payment date; failure to do so will result in the plan being forfeited.
Once payment is successfully made, the ordered vaccination service fee is non-changeable, non-transferable, and/or non-refundable (except for those who are not suitable for vaccination after physician consultation, as mentioned above).
Vaccine availability and pharmaceutical manufacturer brand are subject to availability and the Center reserves the right of final decision.
Vaccination services are not intended for medical diagnosis or treatment.

Union HospitalTerms and Conditions:

- After the successful payment, client will receive a confirmation email from health.ESDlife and client will be contacted by Union Health Maintenance Centre within 2 working day for the confirmation of booking date and time. Client can also query your transaction in the next working day. (General Enquiry Hotline: 2608 3170)
- Client must present his / her identity document and the print-out of order confirmation email on the appointment day.
- The healthcheck package is valid for six months (from the date of purchase) or it will be forfeited.
- The purchase cannot be altered or refund after the confirmation of order.
- Under normal circumstances, all medical reports will be ready around 7-10 working days (excluding Sunday and public holiday). A delay in processing some requests due to the following reasons: 1. Some items take more turnaround time for test; 2. The specific selective timslot requested by client.
- All health screening test or examination is for the purpose of medical diagnosis or treatment
