Terms of reservation and validity:
After receiving the confirmation email of successful payment from the FWD MAX Health Shop Site owned by FWD Life Insurance Company (Bermuda) Limited (incorporated in Bermuda with limited liability) (“FWD”) and operated by ESD Services Limited (“ESD”, “health.ESDlife”) (“FWD MAX Health Shop”), customers should contact EC Healthcare by phone (Tel: (852) 2120 6185) or via WhatsApp (Tel: (852) 4625 9363) to make an appointment within office hours. Please note that the opening hours of the medical centres may be subject to change without prior notice. Please make appointments for any medical or wellness services at least 2 working days in advance.
After making the appointment, if any changes to the appointment details, including change in time, location etc., are required, customers must contact EC Healthcare and arrange by themselves. If customer makes an inquiry or appointment with EC Healthcare via instant message or email, representative from EC Healthcare will respond within 1 working day.
The confirmation email of successful payment issued after purchase of the service is valid for 3 months. Customers must receive the relevant service within 3 months from the date of receipt of the confirmation email of successful payment. The confirmation email will be void after expiry.
[Only applicable to vaccination service] If a customer purchases a vaccine package where multiple doses are included, the 3-month validity period of the above-mentioned confirmation email of successful payment would only be applicable to the time of receiving the first dose of vaccine, meaning that the customer should contact EC Healthcare within 3 months after receiving the email to make an appointment for the first dose of vaccine, and can arrange the schedule for receiving the remaining doses with EC Healthcare according to the recommended schedule.
Other terms:
All service recipients must present their proof of identity and confirmation email of successful payment upon registration at the centre before receiving the service.
If the services are purchased by parents or guardians for their children under 18 years old, service recipients must be accompanied by their parents or guardians to the centre. The parents or guardians must sign the consent form and present their proof of identity. Services will be provided after verification. In addition, if the child is going to receive vaccination service, he/she must be accompanied by a parent or guardian and present his/her vaccination record or immunization record card when receiving vaccination.
Under normal circumstances, after a customer has made a purchase, amendment to, cancellation of or transfer to a third party of and/or submission of refund request for the service(s) will not be allowed.
Report (applicable to health check-up only):
• Under normal circumstances, electronic medical reports will be sent to customers within 7 to 10 working days (excluding Saturdays, Sundays and public holidays) after the date of check-up.
• If a customer would like to request a printed report, he/she could contact EC Healthcare via the aforementioned means. The cost for the first mailing of a printed report will be waived, but customers should bear the risks associated with mailing the printed report.
• Mailing of printed reports is limited to addresses within Hong Kong.
• If a customer requests another mailing after EC Healthcare has sent the printed report, the customer shall be responsible for the relevant costs.
Notes for vaccination (only applicable to vaccination):
• Customers should seek independent medical advice for their suitability for vaccination.
• The customer’s suitability for the vaccination must be assessed by medical staff prior to the injection, and the injection procedures must be performed by medical staff.

Any inquiries or claims after the health check-up/service should be submitted to and resolved directly with the service provider.
• A customer’s suitability for a service is assessed by and subject to the final decision of the service provider, medical centre or merchant. If the medical staff determines that the customer is not suitable for the service, the customer can submit a refund request to the FWD MAX Health Shop, and provide the evidence of the service provider deeming the customer not suitable for the service. FWD MAX Health Shop reserves the right to deduct doctor consultation fee, administration fee and/or any other charges incurred from such service collected by the service provider from the refund amount, and FWD reserves the right of final decision on matters of refund.
• If a customer purchases a service at discounted price and opts for additional services or other services (if any), provided by the service provider and/or its healthcare network team, the customer will have to pay the extra fees to the service provider depending on the service selected.
• The information, prices, procedures and arrangements of the services displayed on this website are provided by the service provider and its healthcare network team and are for reference only, and the decisions made by the service provider shall be final and binding.
