The Hong Kong Pharmaceutical Care Foundation, established in 2007, is a government-registered non-profit charitable organization founded by pharmacists. It focuses on providing pharmaceutical care services to elderly residents in care homes and improving medication-related issues. The main services include visiting pharmacist services, computerized medication management, automated packaging and electronic dispensing systems, as well as medication knowledge and management training. The foundation uses professional pharmaceutical services to establish a safe and appropriate medication management system, assisting care homes in managing medication storage, distribution, and disposal according to guidelines while maintaining complete and accurate medication records. Pharmacists also assess the medication safety of residents by reviewing potential drug interactions, duplicate medications, and side effects to reduce medication incidents. Additionally, the foundation promotes medication education by providing various training programs for care staff, nursing assistants, registered nurses, and social workers to enhance medication safety awareness.

Terms of reservation and validity:
After receiving the confirmation email of successful payment from the FWD MAX Health Shop Site, owned by FWD Life Insurance Company (Bermuda) Limited (incorporated in Bermuda with limited liability) (“FWD”) and operated by ESD Services Limited (“ESD”, “health.ESDlife”) (“FWD MAX Health Shop”), customers should contact HKPCF via WhatsApp ((852) 4645 2787) or email (info@pcfhk.org). Please note that the opening hours of HKPCF are subject to change without prior notice. Please make appointments for any medical or wellness services at least 3 working days in advance.
After making the appointment, if any changes to the appointment details, including time, etc., are required, customers must contact HKPCF and make the arrangements themselves.
The confirmation email of successful payment issued after purchasing the service is valid for 6 months. Customers must use the relevant service within 6 months from the date of receipt of the confirmation email for successful payment. The confirmation email will become invalid after expiration.
Other terms:
All service recipients must present their proof of identity and confirmation email of successful payment upon registration at the centre before receiving the service.
If the services are purchased by parents or guardians for their children under 18 years old, service recipients must be accompanied by their parents or guardians to the centre. The parents or guardians must sign the consent form and present their proof of identity. Services will be provided after verification. In addition, if the child is going to receive vaccination service, he/she must be accompanied by a parent or guardian and present his/her vaccination record or immunization record card when receiving vaccination.
Under normal circumstances, after a customer has made a purchase, amendment to, cancellation of or transfer to a third party of and/or submission of refund request for the service(s) will not be allowed.
Report (applicable to health check-up only):
- Under normal circumstances, HKPCF will only provide health screening report, and to present to the customer at immediate at the service center after health screening.

- A customer’s suitability for a service is assessed by and subject to the final decision of the service provider. If the service provider determines that the customer is not suitable for the service, the customer can submit a refund request to the FWD MAX Health Shop, and provide the evidence of the service provider deeming the customer not suitable for the service. FWD and ESD reserve the right to deduct administration fee and/or any other charges incurred from such service collected by the service provider from the refund amount, and FWD and ESD reserves the right of final decision on matters of refund.
- If a customer purchases a service at discounted price and opts for additional services or other services (if any) provided by the service provider, the customer will have to pay the extra fees to the service provider depending on the service selected.
