HealthMutual Group (“HMG”), a healthcare scheme and product management company, commenced its operation in 2014. HMG integrates professional actuarial knowledge, and medical expertise into healthcare management services.
Terms
• After receiving the confirmation email of successful payment from the FWD MAX Health Shop Site owned by FWD Life Insurance Company (Bermuda) Limited (incorporated in Bermuda with limited liability) (“FWD”) and operated by ESD Services Limited (“ESD”, “Health.ESDlife”) (“FWD MAX Health Shop”), customers should contact HMG by phone (Tel: (852) 8120 9066) ) or WhatsApp (Tel: (852) 5951 4452) to arrange the redemption for the Faecal Occult Blood Test (“FOBT”) kit, complete the procedures for collection of the specimen and contact HMG for arrangement for returning the specimen within 1 month.
• After arranging the redemption with HMG, if any changes to the redemption details, including change in address, email address, available time for parcel collection etc., are required, customers must contact HMG and arrange by themselves.
• The confirmation email of successful payment issued after purchase of the service is valid for 1 month. Customers must contact HMG for redemption, complete the procedures for collection of the specimen and contact HMG for arrangement for returning the specimen within 1 month (from the date of payment confirmation). The confirmation email will be void after expiry.
• The person intending to receive this service must understand and accept the risk of such service, such as the accuracy of the FOBT. He/she is advised to seek prior independent medical advice based on his/her personal condition to assess the suitability of receiving the service.
• Customer shall redeem the test kit, provide the copy of the confirmation email of successful payment as a proof and provide the personal information of the test kit user for registration based on the redemption steps as listed in product/service page. The test kit user should fill in and sign the consent letter, collect the specimen, then contact HMG for arrangement for returning the specimen and the signed consent letter.
• By signing the consent letter to HMG, the test kit user agrees for HMG and its healthcare network team to use his/her personal data for the purpose of FOBT result.
• The FOBT kit cannot be exchanged for cash, resold or exchanged for other medical services. If the test kit is lost, stolen or damaged, it will not be re-issued.
• FWD reserves the right to amend the terms and conditions of the service or terminate or discontinue the service at any time without prior notice. All decisions made by FWD shall be final and binding.

• All health check-up/health screening services are neither for the purpose of medical diagnosis nor medical treatments. If you show signs of symptoms of disease, please consult a doctor with recognized qualifications immediately for diagnosis and treatment.
• The merchant is the service provider of this service/product. Health.ESDlife and FWD are not the service provider of this service/product. Health.ESDlife and FWD are irresponsible to any loss, injury or law action caused by using this service/product. Any claims and inquiries should be addressed to the respective merchant.
• This service is provided by HealthMutual Group Limited, a third-party merchant. Terms and conditions apply. FWD makes no representation, warranty or undertaking as to the availability and quality of the service. FWD shall not accept any responsibility or liability for their services, opinions, treatment, negligence, omission or failure to act by such third-party service providers. FWD reserves the right to replace any of such service provider or cease and/or suspend the provision of such services without prior notice. This service is optional. The provision of such service and your acceptance of the same shall constitute a separate contract between you and the third party service provider. Any inquiries or claims after the health check-up/service should be submitted to and resolved directly with the service provider.
• After a customer’s order is confirmed, a refund will not be made under any circumstances. If any defective or missing item is found, please contact the service provider directly for further assistance.
• If a customer purchases a service at discounted price and opts for additional services or other services (if any) provided by the service provider, the customer will have to pay the extra fees to the service provider depending on the service selected.
• The information, prices, procedures and arrangements of the services displayed on this website are provided by the service provider and are for reference only, and the decisions made by the service provider shall be final and binding.
