By adopting the most advanced equipment and following the strictest industry guidelines, we are the pioneer in Hong Kong to implement “completely unbroken cold chain to point of vaccines administration” to give peace of mind to every client.
We adopt a series of high standard equipment to ensure that the vaccines are kept within the temperature range recommended the by World Health Organization (+2°C to +8°C), and let customers feel at ease by providing high quality vaccines and a team of medical professionals:
• 24-hour temperature monitoring system: We adopted 24-hour temperature-monitored vaccine refrigerators, portable vaccine carriers, vaccine vial monitors (VVM), and thermal imagers, to ensure quality and potency of our vaccines.
• Vaccine quality and authenticity: We only administer vaccines from American and European pharmaceutical suppliers, distributed by authorized distributor.
• Team of Medical Professionals: Our qualified medical professional team follows all prescribed procedures in vaccine administration.
Terms & Conditions:
• After receiving the confirmation email of successful payment from the FWD MAX Health Shop Site owned by FWD Life Insurance Company (Bermuda) Limited (incorporated in Bermuda with limited liability) (“FWD”) and operated by ESD Services Limited (“ESD”, “health.ESDlife”) (“FWD MAX Health Shop”), customers should contact Dedicare Medical Centre by phone (Tel: (852) 2468 2248) or via WhatsApp (Tel: (852) 9298 1308) or WeChat (ID: dedicarehk) to make an appointment and receive the service within 3 months.
• After making the appointment with Dedicare Medical Centre, if any changes to the appointment details, including change in time, location, etc., are required, customers must contact Dedicare Medical Centre and arrange by themselves.
• All service recipients must present their proof of identity and confirmation email of successful payment upon registration at Dedicare Medical Centre before receiving the service.
• If the service recipient is under the age of 18, he or she must be accompanied by a parent or guardian and present his/her vaccination record or immunization record card when receiving vaccination. The parent or guardian must sign a consent form for vaccination on-site and present his/her proof of identity for registration. Service will only be provided after verification.
• The confirmation email of successful payment issued after purchase of the service is valid for 3 months. Customers must receive the relevant service within 3 months (from the date of payment confirmation) and should contact Dedicare Medical Centre no less than 3 working days in advance to make an appointment. The confirmation email will be void after expiry.
• If a customer purchases a vaccine where multiple doses are recommended, we suggest that the customer purchase the number of doses required in one go. Under such circumstances, the 3-month validity period of the above-mentioned confirmation email of successful payment would only be applicable to the time of receiving the first dose of vaccine, meaning that the customer should contact Dedicare Medical Centre within 3 months after receiving the email to make an appointment for the first dose of vaccine, and could arrange the schedule for receiving the remaining doses with Dedicare Medical Centre according to the recommended schedule. Please note that customers are required to make appointment for each dose of vaccine no less than 3 days in advance.
Notes for vaccination:
• Customers should seek independent medical advice for their suitability for vaccination.
• The customer’s suitability for the vaccination must be assessed by medical staff prior to the injection, and the injection procedures must be performed by medical staff.
• The number of doses is the general recommended dosage and is for reference only. The actual number of doses needed may be adjusted by medical staff based on the age of the service recipient and other factors.
• Under normal circumstances, if the customer has already received the first dose of vaccine, amendment or cancellation is not allowed, and the remaining doses are neither transferable nor refundable.

The provision of such service and your acceptance of the same shall constitute a separate contract between you and the third party service provider. Any inquiries or claims after the health check-up/service should be submitted to and resolved directly with the service provider.
• A customer’s suitability for a service is assessed by and subject to the final decision of the service provider, medical centre or merchant. If the medical staff determines that the customer is not suitable for the service, the customer could submit a refund request to the FWD MAX Health Shop, and provide the proof of the service provider deeming the customer not suitable for the service.
• In case a customer fails to receive the service he/she purchased within the validity period due to reasons caused by the service provider, the customer could submit a refund request to the FWD MAX Health Shop and provide relevant proof (if any).
• FWD MAX Health Shop reserves the right to deduct doctor consultation fee, administration fee and/or any other charges incurred from such service collected by the service provider from the refund amount, and FWD reserves the right of final decision on matters of refund.
• If a customer purchases a service at discounted price and opts for additional services or other services (if any) provided by the service provider, the customer will have to pay the extra fees to the service provider depending on the service selected.
• The information, prices, procedures and arrangements of the services displayed on this website are provided by the service provider and are for reference only, and the decisions made by the service provider shall be final and binding.
